Comparison of Different Fixation Techniques of the Long Head of the Biceps Tendon in Superior Capsule Reconstruction for Irreparable Posterosuperior Rotator Cuff Tears: A Dynamic Biomechanical Evaluation

Background: In the past decade, superior capsular reconstruction has emerged as a potential surgical approach in young patients with irreparable posterosuperior rotator cuff tears (RCT) and absence of severe degenerative changes. Recently, use locally available biological viable autografts, such long head biceps tendon (LHBT) for SCR emerged, promising early results. Purpose/Hypothesis: The purpose this study was to investigate effect using LHBT capsule on shoulder kinematics, along different fixation constructs dynamic biomechanical model. authors hypothesized that each 3 proposed techniques would restore native joint including glenohumeral translation (ghST), maximum abduction angle (MAA), cumulative deltoid force (cDF), subacromial peak contact pressure (sCP). Study Design: Controlled laboratory study. Methods: Eight fresh-frozen cadaveric shoulders (mean age, 53.4 ± 14.2 years) were tested simulator. Each specimen underwent following 5 conditions: (1) intact, (2) tear (psRCT), (3) V-shaped reconstruction, (4) box-shaped (5) single-stranded reconstruction. MAA, ghST, cDF sCP assessed condition. Results: significantly increased MAA while decreasing ghST compared psRCT ( P < .001 .001, respectively). Clinical Relevance: Using biologically autograft is cost-effective, time-saving, technically feasible alternative capsule, which may result favorable outcomes psRCTs. Moreover, restored biomechanics, help improve migration tears.
